Emergency Shelter Case Manager

Who We're Looking For...

Southwestern Community Services, Inc. is seeking a highly organized and compassionate human services professional to join our Housing Stabilization team in Sullivan County, New Hampshire. This is a full-time, hourly position.

The Emergency Shelter Case Manager will evaluate the needs of program participants with a focus on arranging, coordinating, and monitoring the delivery of individualized services.  Services may include mental and physical health care and substance abuse and recovery.  They will develop and implement comprehensive case plans that promote homeless individuals and families in moving towards long term self-sufficiency and the reduction of housing barriers.  The Emergency Shelter Case Manager will also assist clients in obtaining federal, state, and local benefits and monitor progress towards goals in regularly scheduled meetings.    As part of the SCS team they will collaborate with the Emergency Housing Coordinator to ensure the safety of all clients by enforcing the rules and regulations of the Emergency Housing Program.   This position requires strong written and oral communication skills, a familiarity with area resources, and a knowledge of basic computer programs including Microsoft Excel and Outlook.

Case Management experience preferred.
