Elder Services

Eldercare operates only in Sullivan County. It is designed to secure and maintain maximum independence and dignity for older  persons in their own home environment. Activities are designed to provide a continuum of care with appropriate supportive services and removal of barriers to economic and personal independence.

 

The program offers the following resources: regular home visits, personal home response units, telephone reassurance, well-being checks, procurement of hearing aids, health insurance counseling and assistance, and assistance in obtaining needed benefits such as Medicaid, Fuel Assistance, etc.

Health Insurance Counseling Education Assistance Service (HICEAS)

HICEAS assists seniors with problems concerning Medicare, Medicaid, Medigap and long-term health care. Trained and certified volunteers provide services through SCS offices, senior centers and housing authorities. In some cases, the service is provided in the participant’s home.

Volunteers explain and assist participants with the maze of health care programs and resources available. Participants are assisted in making cost comparisons of insurance plans, sorting medical bills and understanding benefits, assisting when there is suspicion of vendor fraud and other issues. There is no charge for this service but it can save participants a great deal on money.

Senior Companion Program

Senior Companions commit to visit the homebound elderly for twenty hours a week. They spend time with seniors at various activities – reading and writing letters, walking, or pursuing other hobbies. The program’s purpose is to allow the frail senior population to continue living at home by providing emotional and social support. Visits help foster trusting relationships between seniors and their companions as they share life stories, discuss current events or review community resources. The companion receives a small stipend to supplement income.

Sullivan County ServiceLink

The Sullivan County ServiceLink program provides Information and Supported referral for adults 60 and over, adults living with disabilities, their families and caregivers, that empowers them to live as independently and as fully as possible.

Services Include:

  • Education

  • Follow up Services 

  • Support

Inquiries range from issues regarding available housing, medicaid/medicare questions to wellness programs.
