Child Health Support Program
This program is funded by The State of New Hampshire DCYF.
Most referrals are court ordered to provide services to
families who are at risk of being separated. A Child Health Support
Worker is provided to assist with parenting skills, budgeting skills,
supervised visits between parents and their children and an array of other
support services to foster reunification. Support is also provided to
assist with obtaining housing, medical care, obtaining benefits and
assistance with any other helped deemed necessary.
The program also will, on a case by case basis, take a
private referral from lawyers, other Social Service Agency’s and a private
citizen.
All staff have
a bachelor’s degree and/or related experience with children and families.
The current staff has been with the program for over 6 years.
Transportation Services
This court ordered service is provided to children and extended family who
need assistance in getting to medical appointments, therapy appointments ,
home visits as well as supervised visits. We travel throughout the State of
New Hampshire as well as some out of state travel when needed.
We also have the ability to provide this
service as a private contractor with Schools and other Social Service
Agencies throughout the State.
All our drivers use their own vehicles and
are insured as required by the State of New Hampshire’s Department of Health
and Human Services.
Home Based Therapeutic Service
This program is a comprehensive program
serving families of New Hampshire. Referrals come from DCYF or Juvenile
Justice.
The Home Based program includes in home interventions to:
Strengthen and support the family, assist with family reunification, prevent
placement of a child and to intervene when a crisis arises.
The goals of the program shall be to:
Ensure the safety of children and families
Improve interpersonal relationships and communication within the family
Prevent the placement of a child in out of home care
Improve each child's well being in the home and community
